We Honor Veterans Campaign Fact Sheet – An explanation of how hospices can heighten their visibility in the community through this national campaign, launched in October 2010.
Military History Checklist -- A form to help secure veteran-centric information upon admission to hospice.
Military History Checklist Guide – A guide to be used with the checklist to help hospices identify their veteran patients, evaluate the impact of the military experience and determine any benefits to which the veteran and surviving dependents may be entitled.
