The ideal candidate will be compassionate, knowledgeable, highly motivated and able to work independently, as well as be an integral part of the interdisciplinary team. He/She must demonstrate excellent assessment, communication, interpersonal and customer service skills.
·Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment
·Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties
·Completes the initial grief risk assessment of patients and families.
·Maintains clinical records on all patients referred to social work.
·Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
·Provides information to patients and families/caregivers and community agencies.
·Participates in the development of the plan of care and attends case conferences weekly.
·Assists in provision of bereavement care to families.
Skills and Experience:
·A minimum of 1 year Hospice and/or Home Health experience
·Licensed driver with an insured automobile
·Excellent clinical skills
·Strong computer skills; electronic medical record experience preferred
·Familiarity with Jewish holidays, culture and customs
·MSW from school accredited by the Council on Social Work Education
·LSW [New Jersey State License] required; LCSW preferred
